A Fax cover sheet is the first page that is delivered prior to the document that is being faxed to the recipient. This is an essential document because it gives the person receiving it useful information. It lets the person handling faxes know what the next pages in the fax are about, who it’s from, and the priority level. Although some companies do not require a cover sheet, omitting it can result in the fax you send being placed as a low priority or even lost.

The quality of your fax cover sheet can have an impact on how efficiently your transmission is handled.

Fax Cover Sheet Examples and Templates

Once an excellent and effective cover sheet is created it can be saved as a Word and/or PDF document as a template. How To Write an Effective Attention-Getting Fax Cover Sheet

The purpose of the cover sheet is to tell the recipient who the fax is from, who should receive it, and what it’s about. This is vital information, but how do you make it stand out? It all begins with a well-planned fax cover letter that can be converted to a template. The key is to write a cover sheet that looks professional, is well-organized, and offers relevant information for the processor. Some offices put one or two people in charge of receiving and forwarding faxes, while others let everyone help themselves. To make your sheet look professional and worthy of a second glance, here are the foolproof steps for creating the best possible fax cover page.

  • Add letterhead if you have it. Letterhead makes the cover sheet look official and more important. It can be for a company or for an individual and it must include the name of the company, address, telephone number, fax number, and email address in that order
  • Fill in who it’s going to. Most templates begin the recipient information with the heading To: List the company name and the name of the person you’re sending the fax.
  • Complete the sender’s information. Directly across from To: is the heading From: on the same line. Fill in your name
  • Fax and Page information. The next line down is the heading Fax: Fill in the fax number of the sender. Directly across is the heading Pages: Fill in the number of pages contained in the fax you’re sending and include the cover sheet in the count. This ensures the recipient knows how many pages were sent.
  • Phone and Date information. On the next line down the heading Phone: is where you list the sender’s telephone number. Directly across is the heading Date: and this is where you enter the date that you sent the fax.
  • Subject and cc. On the next line, the heading Re: is where you fill in the subject, or what the fax is about. On the same line, directly across is cc: and this is where you list the name of any other person to whom the fax is being sent.
  • Priority line. The following line is separated by a line above and below to denote importance. This is the priority of the fax. Your choices are Urgent, Confidential, For Review, Please Comment, Please Reply, and Please Recycle. Each has a box in front of it for selection. Check one or more of the most applicable priorities.
  • Comments. The final line is reserved for your message. It begins with the heading Comments: Here you may add informational text or images to let the recipient know more details about the fax.
